Does anyone know about a company who customizes project enclosures (machining, pad printing, etc.) in small quantities for a low price? Any replies are appreciated!

Try Polycase.

I bought one there as a sample and was quoted about $150 with machining. I didn't get a quote for printing but according to their website, it's a service they offer.

Does that mean it is $150 per box, or is that the startup fee?

OOPS, sorry! The price I was quoted was for 10 enclosures. You could get only one made but I think the cost is about $60.00.

Radioshack sells premade enclosures in quite a few sizes. If you're on a budget like me, you might want to check that out.

Here is a place to get deals on boxes and a lot of other parts too. http://www.allelectronics.com/ I buy a lot of stuff here and they are great to deal with.
